Permit to Work for Maintenance staff/contractors.

This form is to be written in advance of the contractor/engineer arriving and should be completed in their presence. A short verbal induction should be given to local procedures.

This forms allows authorised access to an engineer to perform service/repairs. This forms serves to highlight to the engineer that they are entering a CL2 facility and demonstrates that risks have been assessed and there are procedures to follow.
